Efficacy
In experimental and therapeutic conditions, betamethasone valerate exerts anti-inflammatory, anti-allergic, and anti-proliferative effects.
When applied locally, it demonstrates anti-inflammatory action, effects on the epidermis, stratum corneum of the skin, anti-allergic action, and reduction of contact sensitivity.
In the Mackenzie-Stout test, which correlates with the therapeutic efficacy of local corticosteroids, 23 esters of betamethasone were studied. The vasoconstriction activity of betamethasone valerate, relative to fluocinolone acetonide, was 360, while fluocinolone acetonide had an activity of 100.
Mechanism of Action
The mechanism for the anti-inflammatory, anti-proliferative, and immunomodulatory actions of all glucocorticoids can be schematically represented in a simplified form as follows:
Glucocorticoid molecules form complexes with corticosteroid receptors in plasma and are transported into the cell nucleus, where they bind to specific GRE (glucocorticoid response elements) genes. This induces transcription of specific mRNA molecules, leading to the synthesis of lipocortin proteins on ribosomes. Lipocortins slow down reactions caused by physical, chemical, toxic, or immunogenic impacts or microbial pathogenic agents. They act between phospholipase A2 and membrane phospholipids, releasing arachidonic acid.
This delay or slowdown in the release of arachidonic acid normalizes, reduces, or blocks the synthesis of eicosanoids, including prostaglandins, thromboxanes, leukotrienes, and prostacyclins, which are inflammatory mediators influencing blood vessels, cell membranes, leukocytes, macrophages, chemotaxis, and migration.
Additionally, glucocorticoids have an anti-mitotic effect and slow the synthesis of nucleic acids and proteins. Their immunomodulatory and anti-allergic effects involve interaction with B-cells, T-cells, and Langerhans cells, slowing the movement of antigens and their antagonistic effects on the synthesis and function of interleukins 1, 2, and other cytokines.
Pharmacokinetics
When applied topically and for a limited period, the betamethasone-containing drug does not result in systemic absorption of a therapeutically significant amount of the substance.
With prolonged use, or when applied to large areas, especially in damaged skin, or with occlusive dressings, significant systemic absorption may occur.
In vivo, betamethasone valerate is rapidly hydrolyzed by esterases into betamethasone alcohol.
Betamethasone metabolism has been studied on healthy volunteers and patients under high therapeutic doses. Approximately 70% of the dose is excreted in the urine within 48 hours, with 15-30% found as an indirect fraction. Six metabolites and unchanged betamethasone were identified. The transformation involves hydroxylation and reduction at various positions on the molecule.
Indications
Treatment of psoriasis of the scalp and other non-infectious inflammatory, allergic, or pruritic skin conditions where symptomatic use of potent corticosteroids is indicated.
Contraindications
Soderm® solution is contraindicated in:
- Specific skin conditions (tuberculosis, syphilis-related skin manifestations)
- Rosacea
- Acne
- Chickenpox
- Reactions to vaccinations
- Pruritus ani (anal pruritus)
- Perioral dermatitis
- Infectious skin diseases caused by viruses, bacteria, or fungi
- Hypersensitivity to betamethasone valerate or any components of the product.
- Not for use in children under 1 year of age.
- Not for use in generalized psoriasis in the chronic or stationary stage.
- Do not apply to infected or erosive, weeping areas of skin or skin cracks and ulcers.
- The facial skin is particularly sensitive; avoid corticosteroid therapy in the facial area.
- Avoid applying to the eyelids or the eye area to prevent glaucoma and cataracts under certain conditions.
- Prolonged use (more than 3-4 weeks) or high doses (large areas) with occlusive dressings should be avoided.
- Avoid applying to the chest and breasts of lactating women.
